The Effect of Body Mass Index on Sacral Hiatus Location and Size of Pelvis

Abstract
Objective : To identify the effect of body mass index on sacral hiatus location measured by ultrasound and pelvis size assessed with plain X-ray.

Materials and Methods: Prospective data were collected from 88 outpatients who underwent caudal epidural injection using ultrasonography. We grouped the patients into 4 according to body mass index: underweight, normal, overweight, and obese groups. We measured the depthof sacrococcygeal ligament, the vertical length of sacral hiatus, the depth of sacral hiatus with transverse and sagittal views, and the thickness and width of sacrococcygeal ligament using ultrasonography. We also used the plain X-ray image of the pelvis to measure the height of the sacrococcyx, the false pelvis, and the true pelvis.

Results: In longitudinal and transverse section, there were significant differences between the obesity group and the other 3 groups regarding the distance from skin to sacrococcygeal ligament and distance from skin to sacral hiatus (p£¼0.05). In transverse section, the thickness of sacrococcygeal ligament was significantly increased in the obese group, compared to normal group (p£¼0.05). The values measured by plain X-ray had no correlation with groups classified by body mass index (p£¾0.05).

Conclusion: When a needle is inserted at the sacral hiatus point using the conventional blind method, greater depth of injection is needed for obese patients.
